Lab-on-a-Chip Device Developed to Screen for Oral Cancer

This year about 34,000 Americans will be diagnosed with oral or throat cancer. These types of cancer will result in over 8,000 deaths this year, or about 1 person every hour, 24 hours a day, 7 days a week. Of the 34,000 newly diagnosed oral cancer patients, only half will be alive in 5 years. The prognosis for this group of cancer patients has not significantly improved over the last few decades. Worldwide the problem is much greater, with over 350,000 new cases each year.
The reason for the high mortality rate here is that oral cancer is typically discovered only in the late stages of its development. Once discovered, oral cancer is particularly dangerous because it tends to produce second site, primary tumors. Unfortunately, for patients that do survive a first encounter, they have up to a 20 times higher risk of developing a second type of cancer. There are many types of oral cancers, but 90% fall into the type of squamous cell carcinomas.

Many oral cancer patients are diagnosed during a dental exam. While there are some tools used by dentists to help diagnose the disease, most of the tools lack the sensitivity and selectivity to make this diagnosis reliable or are associated with side effect for the patient. New methodologies that can be used at the point-of-care are desperately needed to help improve the diagnostic and prognostic capabilities for this area. Follow up visits which serve to follow the progression of the disease after treatment are one area that may be particularly well suited for a lab-on-a-chip portable oral cancer screening unit. We are now involved in an active collaboration supported by the National Institute of Dental and Craniofacial Research Division of the NIH that pairs the McDevitt lab at the University of Texas at Austin with the labs of Dr. Spencer Redding and Dr. Chih-Ko Yeh at the University of Texas Health Science Center at San Antonio.

Texas Researchers Aim to Use Saliva To Diagnose Health and Disease

AUSTIN, Texas—Innovative saliva-based health diagnostic tools will be developed by researchers at The University of Texas at Austin through a $6 million, multi-institutional grant from the National Institutes of Health (NIH). Saliva—with its slimy mix of proteins, hormones and antibodies—can tell a lot about a person’s health, and it is much easier and less painful to collect than blood. In the last decade, saliva has been advocated as a non-invasive alternative to blood as a diagnostic fluid. However, use of saliva has been hindered by the inadequate sensitivity of current methods to detect the lower salivary concentrations of many constituents compared to serum. Furthermore, developments in the areas related to lab-on-a-chip systems for saliva-based point of care diagnostics are complicated by the high viscosity and heterogeneous properties associated with this diagnostic fluid. The biomarker C-reactive protein (CRP) is an acute phase reactant and a well-accepted indicator of inflammation. Numerous clinical studies have established elevated serum CRP as a strong, independent risk factor for the development of cardiovascular disease (CVD). full-text article

Development of a Miniaturized CD4 Counting System for HIV Monitoring in Resource Scarce Settings, Public Library of Science,  2005, 2(7), 0001-0010.

Here we describe the development of a prototype for a simple, rapid, and affordable method for counting CD4 lymphocytes. Microliter volumes of blood without further sample preparation are stained with fluorescent antibodies, captured on a membrane within a miniaturized flow cell and imaged through microscope optics with the type of charge-coupled device developed for digital camera technology. An associated computer algorithm converts the raw digital image into absolute CD4 counts and CD4 percentages in real time. The accuracy of this prototype system was validated through testing in the United States and Botswana, and showed close agreement with standard flow cytometry ( r = 0.95) over a range of absolute CD4 counts, and the ability to discriminate clinically relevant CD4 count thresholds with high sensitivity and specificity. full-text article

We report here the adaptation of our electronic microchip technology towards the development of a new method for detecting and enumerating bacterial cells and spores. This new approach is based on the immuno-localization of bacterial spores captured on a membrane filter microchip placed within a flow cell. A combination of microfluidic, optical, and software components enables the integration of staining of the bacterial species with fully automated assays. full-text article

The development of a micromachined fluidic structure for the introduction of liquid samples into a chip-based sensor array composed of individually addressable polymeric microbeads is presented. The micromachined structure consists of micromachined storage cavities combined with a covering glass layer that confines the microbeads and fluidic channels. In our sensor array transduction occurs via optical (colorimetric and fluorescence) changes to receptors and indicator molecules that are covalently attached to termination sites on the polymeric microbeads. Spectral data are acquired for each of the individual microbeads using a charged-coupled device (CCD) allowing for the near-real-time analysis of liquid sample. Hence the micromachined fluidic structure must allow for both optical access to the microbeads and fluid flow through the micromachined cavities that serve as the microreactors/analysis chambers. One of the key parts of the structure is a passive fluid introduction system driven only by capillary force. This simple means of fluid introduction realizes a compact device. The capillary flow on the inlet channel has been studied, and the responses of the microbeads (alizarin complexone) to a liquid sample have been characterized. Herein we report the combination of a library of resin-bound sensors along with a multicomponent sensor array. This novel combinatorial array sensor system shows selectivity for nucleotide phosphates in solution. The design of the anchored receptor includes a 1,3,5-trisubstituted-2,4,6-triethylbenzene scaffold coupled with peptide libraries. Each chemosensor is placed into a micromachined cavity within a silicon wafer, and the optical changes observed by a charged-coupled device result in near-real-time digital analysis of solutions. The sensations of smell and taste result from a series of specific and nonspecific molecular recognition events that take place in parallel. In a few cases there are receptors that are specific for individual analytes. However, most tastants and odorants are identified through a composite of responses from nonspecific interactions. The pattern created by the simultaneous response of these receptors is specific for a particular set of stimuli. For example, the mammalian tongue has "taste buds" which respond to four taste catagories: sweet, sour, salty, and bitter. Methods capable of forming highly organized monolayers on top of YBa 2 Cu 3 O 7 (YBCO), a high- T c superconductor, have been identified and are described for the first time. Here, grazing reflectance infrared fourier transform spectroscopy (GRIFTS) is employed to evaluate the degree of order for these monolayer structures. Through these investigations, it is found that while octadecylamine forms a well-ordered, crystalline-like monolayer on the surface of c -axis-oriented YBa 2 Cu 3 O 7 thin films, the same reagent adsorbed onto polycrystalline YBa 2 Cu 3 O 7 pellets affords disordered, liquid-like monolayers...full-text article

Electrochemical techniques are exploited to fabricate conductive polymer/high- Tc superconductor bilayer structures. Scanning electron microscopy, atomic force microscopy, and electrochemical techniques are utilized to characterize the electrodeposition of polypyrrole layers grown onto YBa~Cu3076 films. In such hybrid polymer/superconductor systems, it is found that, when the polymer is oxidized to its conductive state, the transition temperature (Tc) and critical currents (J,) of the underlying superconductor film are suppressed. Reversible modulations of the transition temperatures of up to 50 K are noted for these structures. Upon reduction of the conductive polymer layer back to its nonconductive form. both T, and J, are found to return to values close to those acquired for the underivatized YBa2Cu3076 film. full-text article
